Lost Copy Card Form

If you have lost your MCard, the MCard website has information about what to do.

If you have lost a $2, $5 or $10 fixed amount copy card, we are unable to replace the card.

If you have lost an MLibrary Copy Card charged to a shortcode, please fill out the form below to have it canceled. To activate another MLibrary Copy Card, please use the card charge form.
